MEDICAL INSURANCE & LONG TERM CARE
MEDICAL:
MEDICARE ELIGIBLE
The HCMA currently endorses Anthem Blue Cross & Blue Shield
as the
carrier for its endorsed programs of health insurance for the Medicare eligible
members and their spouses. There are three plans in place:

The High Option Plan 81 with Major Medical is
currently frozen and not accepting new members. This plan is deemed to be
“CREDITABLE” coverage.

Medicare Supplement Plan F with an Unlimited
Prescription Drug Rider otherwise not available to members on a stand
alone basis. This plan is deemed to be “CREDITABLE” coverage.

Medicare Supplement Plan F without Prescription
Drug Coverage. This plan is NOT deemed to be “CREDITABLE” coverage.
DENTAL
The HCMA currently endorses two carriers and plans for members, their
families and staff. Both are VOLUNTARY and not subject to normal issue
and participation requirements.

The CIGNA DMO requires utilization of their
providers with no deductibles or annual caps at extremely competitive rates.

The MetLife plan is a more traditional plan
with deductibles, co-pays and a cap on spending. It does, however, allow for
the utilization of any licensed provider.
Group Term Life
The HCMA currently endorses MetLife. All member physicians and
staff are eligible for one and one half times their salary subject to a maximum
of $300,000. Guaranteed issue for some of the coverage is available for new
entrants to the plan who apply within sixty days of becoming eligible. Current
rates are being reduced by 35%, making the plan extremely competitive,
especially at the earlier ages.
Long Term Convalescent
(LTC)
The HCMA once endorsed CNA for LTC. We have a number of
policies in force and continue to service the policies without problems.
For new applicants we are currently writing with Genworth, MetLife, John
Hancock, Prudential and Massachusetts Mutual while we search for a new
endorsed carrier that will either provide some underwriting advantages or
premium discounts. With nursing home costs in Connecticut already in excess of
$100,000 a year and increasing yearly, protecting the assets you accumulate
from a long term illness not covered by your health insurance or Medicare is
becoming an increasingly important consideration.
DISABILITY &
LIFE INSURANCE DISABILITY (INDIVIDUAL)
Disability
-
The HCMA once endorsed Massachusetts Casualty for individual
disability insurance. The Company was sold but a number of policies are in force
and continue to be serviced.

For new applicants the HCMA endorses Massachusetts Mutual with the
following pluses, making it worthy of consideration:

A strong financial carrier with an “Own Occupation” rider
-
A Group Wrap product and Trust program for protecting your pension
contributions

A 15% premium discount
-
The HCMA also continues to endorse the following coverage through
The
Hartford:

Weekly Disability
-
Office Overhead Insurance
-
Both utilize banded rates and have short
elimination periods and long benefit durations which are not the norm in
today’s marketplace.
PROFESSIONAL LIABILITY
Connecticut Medical Insurance Company (CMIC)

Accidental Death & Dismemberment (AD&D)
The HCMA offers up to $100,000 of AD&D through The Hartford.
